Thank you for your interest in a Healthy Texas! This survey was prepared and is being distributed by Live Smart Texas. Live Smart Texas is a consortium of researchers, prevention advocates, and government employees concerned with improving the lives of Texans at all ages, but with a specific focus on children. The information we are collecting will be used to help coordinate our efforts to keep Texas healthy.
We have generated a list of 7 different questionnaires, each of which is designed to collect information about child obesity, nutrition and physical activity programs and activities in Texas. Please select the type of questionnaire that is applicable for your activity or activities using the brief descriptions listed below. Please complete the full survey for each of your activities. If you do not know an answer, you may choose to skip a question. If necessary, we may need to contact you to clarity or fill in any missing data. Each dataset description should take less than 10 minutes to complete.
We appreciate your participation and thank you for your assistance in improving the health of Texans.
1. Obesity Leadership Survey-this survey is to document people and/or groups who have been active or are influential in the issue of child obesity, nutrition, and/or physical activity. These individuals and/or groups will include legislators, leaders of certain organizations, etc. It may be that these people/groups are not actively conducting an obesity-related program right now, but their support is necessary for this effort to move forward.
2. Obesity Policy and Environmental Change Survey-this survey is to document current policies in place to address child obesity, nutrition and physical activity. Policies include state level policies or protocols, as well as local policies/protocols that might influence school districts or even individual schools.
3. Obesity Program and Intervention Survey-this survey is to document programs and interventions in child obesity, nutrition and physical activity that are on-going in the community and/or clinical settings.
4. Obesity Prevention Research Survey-this survey is to document child obesity prevention research that is being done in Texas. These projects should have external funding, and strong outcome measures.
5. Obesity Instruments and Protocols Survey-this survey is to document instruments and protocols used in the evaluation of child obesity, nutrition and/or physical activity programs and research, especially focusing on those developed or implemented in Texas.
6. Obesity Data Survey-this survey is to document datasets that contain information about child obesity, nutrition and/or physical activity in Texas.

| WIC Farmers’ Market Nutrition Program (FMNP) Regulations SUMMARY: This interim final rule amends the WIC Farmers’ Market Nutrition Program (FMNP) regulations to codify three FMNP nondiscretionary provisions mandated in the Child Nutrition and WIC Reauthorization Act of 2004. The three nondiscretionary provisions include the option to authorize roadside stands, a reduction in the required amount of State matching funds, and an increase in the maximum Federal benefit level. These changes are intended to increase State agency flexibility in managing the Program. The first two provisions became effective on October 1, 2004, while the increased maximum Federal FMNP benefit level was effective as of June 30, 2004. read more ... |

| SNAP, a new name for the federal Food Stamp Program On October 1, 2008, Supplemental Nutrition Assistance Program (SNAP) became the new name for the federal Food Stamp Program, as a result of the recently enacted Food, Conservation, and Energy Act of 2008 (P.L. 110-246), also known as the Farm Bill. Thus begins a new era in nutrition assistance at USDA. The new name more accurately reflects the Program's mission to provide food assistance and nutrition education to assist participants as they move to a healthier lifestyle and self-sufficiency.
